It is required to pile your personal information, such as your address, phone number on the data base. The information you submit may be showed up on the university staff directory. I think you can write down your address and the phone number in Korea. If you have a new address here later, then you can change that information. I guess you may need to submit that information once again after you settle down here. This form seems to be needed regarding my appointment as a new TA in Fall. I would like to know what I should fill in the blanks under Home/Permanent Address, Home Phone and Military Status. Do I have to write down the address and the phone number of my permanent home in Korea, or do I have to leave the blanks empty until I will have an address and a phone after I settle down in the US? I fulfilled my military obligation in Korea, but I think I should make no marks in the Military Status item because I think the military service in the item means only a service in the US military.
