This form seems to be needed regarding my appointment as a new TA in Fall. I would like to know what I should fill in the blanks under Home/Permanent Address, Home Phone and Military Status. Do I have to write down the address and the phone number of my permanent home in Korea, or do I have to leave the blanks empty until I will have an address and a phone after I settle down in the US? I fulfilled my military obligation in Korea, but I think I should make no marks in the Military Status item because I think the military service in the item means only a service in the US military. I would greatly appreciate any comments on my inquiries. Regards, Jeongoo Cheh |
