I worked in the states with J-1 visa last year. I had once extended my visa in Mexico. Are you working at a school? Then you should ask International student/scholar office for details. You can't get US-visa issued in the States. You should go to US consulate in other countries. That's why I went to Mexico (fortunately, I was in Sandiego so it didn't take long to get there) However, you should contact to the consulate and prepare whatever is needed before actually going there. About waiver stuff.... If you are in the LA or washington, you are lucky! Just go to Korean consulate in the LA (Wilshir Bd.) or Korean Embassy in the washington and grab a set of paper for J-1 visa waiver.
