Notice of Intent to File Labor Condition Application form 4. Copies of academic degrees and transcripts, diplomas and/or licenses. 5. A notarized English translation of the above diplomas, if not originally in English. 6. If degree is not from a U.S. College or University, a Credit Evaluation must be obtained to verify that the foreign degree is equivalent to a U.S. degree. 7.
